New Client Registration & Forms - Sandra
New clients of Sandra Paulsen, PhD may register ahead of time by following these procedures.
1. Talk with Sandra first about coming into her practice for treatment, by calling (206) 855-1133. Think about your goals and questions, and we'll speak for a few minutes to decide together whether to schedule. You may wish to discuss:
- Your goals and questions
- Fees and scheduling
2. If we decide to proceed with scheduling a visit, print off the registration form below, complete it and bring it to your initial visit. Be sure to review:
- The office policies regarding emergency procedures
- Charging for missed visits or late cancellations
- Legal limits on confidentialty
